I, the participant’s parent/guardian named above, consent to my child’s participation in the Usman Khawaja Foundation (UKF) Cricket Clinic. I acknowledge and agree that the UKF Cricket Clinic may include, but is not limited to: • recreational sport activities, exercise and training programs; and • strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning, and various other fitness/sport activities. Participants must wear appropriate, comfortable clothing and closed shoes suitable for physical activities. I hereby affirm, and reaffirm each time my child participates in the UKF Cricket Clinic, that: • I will provide UKF with details of any existing medical conditions, and ensure that any relevant medical/emergency plans and required medications (e.g. insulin, Ventolin, EpiPen) are made available. • My child is in good physical condition and does not suffer from any known disability or condition which would prevent or limit their safe participation in this program. • Participation in the UKF Cricket Clinic is voluntary and at my/my child’s own risk. I hereby release the Usman Khawaja Foundation (UKF), and its officers, agents and employees (the “Released Parties”) from any claims, demands, or causes of action arising from voluntary participation in the UKF Cricket Clinic, to the fullest extent permitted by law. I fully understand that participation in the UKF Cricket Clinic may result in injury and I release the Released Parties from any liability, now or in the future, for any condition that may arise directly or indirectly from participation. These conditions may include, but are not limited to, heart attacks, muscle strains or tears, broken bones, shin splints, heat-related illness, injuries to knees, back, or feet, or any other ailment, including death. I understand that photographs and videos taken by UKF during the UKF Cricket Clinic may be used by UKF for social media, its website, and other promotional materials.
